How to fill in each field

Org name & EIN

Include the nonprofit's legal name and tax ID (EIN) and its exempt status.

Common mistake: Omitting the EIN, which donors need to substantiate a deduction.

Amount or description

State the cash amount; for in-kind gifts describe the item (donor sets value).

Common mistake: Assigning a value to in-kind gifts — that's the donor's responsibility.

Goods/services statement

State whether the donor received anything in return (and its value if so).

Common mistake: Leaving out this statement, which is often legally required for deductibility.

Date of gift

Record the date the donation was received for the correct tax year.

Common mistake: Wrong or missing date, complicating the donor's filing.

Frequently asked questions

What should a donation receipt include?+

The nonprofit's legal name and EIN, its tax-exempt status, the donor's name, the date and amount (or description of in-kind gifts), a receipt number, and a statement of whether goods or services were provided in return.

Do nonprofits have to give donation receipts?+

In the US, written acknowledgment is generally required for donations of $250 or more for the donor to claim a deduction. Rules vary by country — check local requirements.

How do I handle in-kind (non-cash) donations?+

Describe the donated item on the receipt but don't assign a dollar value — the donor is responsible for determining and substantiating fair market value.
