How to fill in each field
Org name & EIN
Include the nonprofit's legal name and tax ID (EIN) and its exempt status.
Common mistake: Omitting the EIN, which donors need to substantiate a deduction.
Amount or description
State the cash amount; for in-kind gifts describe the item (donor sets value).
Common mistake: Assigning a value to in-kind gifts — that's the donor's responsibility.
Goods/services statement
State whether the donor received anything in return (and its value if so).
Common mistake: Leaving out this statement, which is often legally required for deductibility.
Date of gift
Record the date the donation was received for the correct tax year.
Common mistake: Wrong or missing date, complicating the donor's filing.
