How to fill in each field
Property & closing
Identify the property and closing date so the commission ties to the transaction.
Common mistake: Omitting the property reference, so the closing office can't match the payout.
Commission basis & rate
Show the sale price, the commission rate, and the gross commission it produces.
Common mistake: Stating only a lump sum with no rate or basis to verify.
Splits
Deduct the brokerage split and any co-broke/referral as separate negative lines.
Common mistake: Netting splits silently, so the agent's take-home can't be audited.
License number
Include your real estate license number as many brokerages require it.
Common mistake: Leaving off the license, delaying the commission payout.
